続発性大網捻転症を合併した腹壁瘢痕ヘルニア嵌頓に対する腹腔鏡下修復術の1例

要旨
患者は87歳，女性．腹痛，嘔吐にて前医受診，腹壁瘢痕ヘルニア嵌頓が疑われ当科紹介．下腹部手術痕に硬結を触知，CTで大網の脱出に付随したRichter型小腸の嵌頓を認めた．小腸が用手還納できたため，待機的腹腔鏡下ヘルニア修復術を施行した．
腹腔内を観察すると，大網が捻転しながらヘルニア囊先端に癒着していた．癒着剝離後，ヘルニア囊はスイスチーズ様に2個のヘルニア門を有していた．捻転大網を切除後，メッシュを4点支持しdouble crown法にて腹壁固定した．術後経過は良好で合併症なく退院した．大網捻転症を合併した腹壁瘢痕ヘルニアに対する腹腔鏡手術は，正確な診断と術後疼痛の軽減に有用な術式と思われた．
