—胃癌における脾摘—胃上部大彎に浸潤する進行胃癌に脾門郭清や脾摘は必要か？

【ポイント】
◆JCOG0110試験の結果により，大彎に浸潤しない胃上部進行癌に対する脾摘は行われなくなった．
◆大彎に浸潤する胃上部進行癌に関しては，いくつかの後ろ向き研究が脾門リンパ節の郭清効果を報告しており，現時点では脾摘を行う施設が多い．
◆脾摘のデメリットを回避する方法として，腹腔鏡やロボットを用いた脾温存脾門郭清が開発され，その安全性を評価する臨床試験が進行中である．
