【幽門保存胃切除術】器械式端端胃-胃吻合—完全鏡視下：ピアス法

　幽門保存胃切除術（pylorus-preserving gastrectomy：PPG）は元来，消化性潰瘍に対して考案された術式^1）であるが，現在は早期胃癌に対する機能温存術式として選択されている．「胃癌治療ガイドライン第5版」においてPPGは，「胃上部1/3と幽門および幽門前庭部の一部を残した胃切除」と定義され，その適応は幽門から腫瘍遠位端までの距離が4 cmないし5 cm以上のT1腫瘍とされている．当科ではこの対象に対して，2005年から上腹部に小開腹をおいて再建を行う腹腔鏡補助下PPGを導入し，2010年からは再建も鏡視下で行う完全鏡視下PPGを行っている．完全鏡視下PPGでは，当初デルタ吻合を採用していたが，2015年より筆者らが開発したピアス法も導入している^2）．
