胆管-胆管吻合

　胆管病変ではない肝移植手術の胆道再建には，胆管-胆管吻合を適応することが可能である．胆管-胆管吻合は，胆汁流出路が生理的で，十二指腸乳頭部Oddi括約筋機能が保たれ，逆流性胆管炎が生じにくい利点があるが，胆管狭窄が多いとされる^1,2）．脳死肝移植で全肝を用いる場合には総肝管-総肝管吻合，生体肝移植の場合にはグラフトの1次分枝より末梢の胆管とレシピエントの胆管との吻合となる（図1）．肝移植のみならず，肝切除手術中の胆管損傷のリカバリーにも応用可能である．
