膵腺房細胞癌の残膵再発切除により長期生存を得た1例

要旨
症例は，38歳男性．入院2年6か月前から上腹部痛を反復していた．4か月前から膵尾部に腫瘍を認め，急速に増大し，手術となった．周囲組織と固着していたため，膵体尾・脾・横行結腸・左副腎・胃後壁筋層（部分）を，一塊に切除した．病理組織学的検査で，12.5×8.0×7.0 cmの腺房細胞癌（以下ACC）と診断された．5年後に膵鉤部に腫瘍が再発し，幽門輪温存膵頭十二指腸切除術を行った．前回と同様の組織像を呈する2.7×2.5×2.5 cmのACCだった．2回目の手術後20年以上を経て，再々発をみない．
