甲状腺転移をきたした胃癌の1例

要旨
症例は54歳，男性．噴門部進行胃癌に対して胃全摘，脾臓・大網切除，D2リンパ節郭清を施行した．術後補助化学療法を行ったが，術後8か月のCTで甲状腺腫瘍を認めた．精査中，腫瘍増大による右反回神経麻痺症状をきたしたため，甲状腺腫瘍の外科的切除を施行した．腫瘍は気管や食道に浸潤し剝離困難であったが，迅速病理診断で腺癌の診断を得て胃癌の転移と考えられたため，可及的な切除にとどめた．永久病理標本でも腫瘍は胃癌の甲状腺転移と診断された．速やかに化学療法を開始したが徐々に進行し，初回手術から約1年9か月，転移性甲状腺癌手術から約9か月で死亡した．胃癌の甲状腺転移は稀な病態であり，文献的考察を加えて報告する．
