乳房再建術を施行した乳腺巨大葉状腫瘍の1例

要旨
症例は35歳，女性．7か月前から左乳房腫瘤を自覚し，急速に増大したため近医を受診．針生検にて葉状腫瘍と診断され，当院紹介となった．視触診にて左乳房ほぼ全体を占める長径20 cmの腫瘤を認め，潰瘍形成を伴っていた．皮膚潰瘍および菲薄化した皮膚を切除するように皮膚切開を加え，乳頭，乳輪を温存し，正常乳腺と1 cm以上の切除マージンを確保し腫瘍切除術を施行した後，深下腹壁動脈穿通枝皮弁による乳房一次再建術を施行した．標本は重さ2,160 g，径20 cmであった．病理診断は境界病変葉状腫瘍で，断端は陰性であった．術後2年間無再発で経過し，良好な整容性が得られている．
