【ポイント】
◆肛門直腸移行部の解剖の理解が重要である．男性の前壁では内肛門括約筋と直腸縦走筋が連続するように直腸尿道筋に移行していること，女性の前壁では内肛門括約筋と直腸縦走筋が収束するように腟後壁に付着していることを認識する必要がある．
◆直腸周囲の筋膜の理解が重要で，直腸固有筋膜，Denonvilliers筋膜（直腸腟中隔），壁側骨盤筋膜などを認識し，剝離を行う必要がある．
◆中部直腸では骨盤神経を確実に温存することが重要で，神経血管束（NVB）および直腸周囲の筋膜をランドマークとした剝離が有用である．
