膵頭十二指腸切除後に経腸栄養を—行う vs 行わない

　膵頭十二指腸切除術（PD）は，高侵襲な術式でありいまだ合併症発生率も高く，消化吸収能も著しく低下する．PD術後の経腸栄養は，栄養状態の改善，術後合併症発生率の低減につながるとされており，教室でもPD施行時には全例ルーチンで腸瘻造設術を併施し術後早期経腸栄養を行っている．腸瘻造設を行うことで術後1日目から経腸栄養を開始することができ，術後合併症により経口摂取が困難な場合でも栄養状態を維持することが可能となる．チューブ留置を外瘻にすることで不快感を軽減でき，下痢をきたした際にも流量調節や止痢剤の併用により投与継続が可能となる．近年その有用性が再認識されつつあり，術後経腸栄養の最新のエビデンス，有用性を再確認するとともに，当施設で行っている術後経腸栄養，臨床試験の結果を含めて報告する．
