当院の成人臍ヘルニアに対する治療成績の検討

要旨
2010年4月から2018年3月の8年間における成人臍ヘルニア待機的手術19例を対象に当院の治療成績を検討した．術式の内訳は開腹単純縫合閉鎖群（SC群）10人，開腹IPOM（OI群）3人，腹腔鏡下IPOM（LI群）6人であった．無再発群と再発群で比較するとBMIにのみ有意差がみられた．ヘルニア門の平均長径（mm）はSC群21.0（10〜32），OI群23.7（15〜40），LI群40.6（24〜63）で，LI群が有意（p＝0.048）に長かった．ヘルニア門が比較的大きくない症例には単純縫合閉鎖よりmeshを用いた再建が有効で，ヘルニア門が大きい症例には腹腔鏡下はIPOMが有効であるが，肥満などのヘルニアの原因に対する治療が必要と考えられた．
