【ポイント】
◆特発性食道破裂は，飲酒後の嘔吐が原因の食道壁全層にわたる損傷であり，速やかな病歴聴取とCT，X線，食道造影で診断をつけることが重要である．
◆発症から24時間以内の時間経過がきわめて重要で，治療方針や術後管理に直結する．
◆内視鏡治療，義歯，PTP製剤，結核などで食道穿孔が起こりうる．縦隔気腫の存在をCTなどで見逃さない．
