小腸・大腸出血／NOMIに対する緊急手術

【ポイント】
◆消化管出血（小腸・大腸出血）は保存治療の進歩が著しく，手術は保存治療に抵抗性のある場合という位置づけにある．
◆出血源の同定が重要．解剖学的構造を理解したうえで治療法を選択する．
◆NOMI（非閉塞性腸間膜虚血症）には特徴的な背景があることが多い．特に循環器疾患や透析，脱水症例の急性腹症では，NOMIを念頭におき，疑うことが重要である．そのうえでの早期診断・治療が救命に繋がる．
