Step1 ポート挿入
　腹腔鏡下括約筋間直腸切除術（intersphincteric resection：ISR）の基本ポート配置は，図1aのような5ポートである．直腸の頭側への牽引が必須であり，われわれは恥骨上に配置した12 mmポートから腸把持鉗子を挿入し，直腸の頭側牽引を行っている．恥骨上ポートは腸間膜脂肪が少ない症例では5 mmポートでもよい．男性・狭骨盤・肥満・巨大腫瘍といった困難症例などで左下腹部にもう1ポート追加する場合は，図1bのようなポート配置となり，この場合は恥骨上ポートから挿入した鉗子はロックアーム^®（システム・ジェーピー）を用いて固定している（図2）．
