炎症性腸疾患におけるストーマ造設と合併症

【ポイント】
◆炎症性腸疾患患者のQOLを保つため，ストーマ造設とその後の適切な合併症の管理がきわめて重要である．
◆潰瘍性大腸炎の術後ではstoma outlet obstruction，壊疽性膿皮症や粘膜皮膚離開などのストーマ合併症に注意する．
◆クローン病の難治性肛門病変にストーマ造設は有効であるが，ストーマ周囲膿瘍や瘻孔，空置された直腸からの癌化に注意が必要である．
