食道癌に対するconversion surgery

【ポイント】
◆Conversion surgery（CS）とは，切除不能病変が導入療法によりdown stagingし画像上切除可能と判断された場合に行う手術を指し，salvage surgeryとは近い概念だがいくつかの相違がある．
◆気管・大動脈へのT4診断には特徴的なCT画像所見があり，また同時に，気管支鏡やPET・MRIなど，ほかのモダリティーも併用した総合的な診断が重要である．
◆cT4b食道癌において，化学放射線療法や3剤化学療法による導入治療後に根治切除が達成できれば比較的良好な予後が期待できるというエビデンスが本邦から出されつつある．
◆食道癌に対するsalvage手術においては，気管血流の温存，再建臓器への放射線の影響，胃管気管瘻の予防などに留意する必要がある．
