胃癌に対するconversion surgery

【ポイント】
◆胃癌に対するconversion surgeryとは，多臓器転移などによりR0手術が不可能と判断されたcStage Ⅳに対して，化学療法によりR0手術が可能と判断されるまで奏効し，治療方針を外科的介入（surgical intervention）にconvertするものである．
◆Conversion surgeryの適格規準は，以下の3点が挙げられる．①1次治療により一定の抗腫瘍効果が得られている．②1次治療による奏効が維持され増悪傾向がない．③R0手術が可能と判断できる．
◆Conversion surgeryは術後合併症の頻度の検討から比較的安全に行うことが可能で，治療成績の検討からR0手術の達成により飛躍的な予後改善が期待できる．
