局所進行肝門部領域胆管癌に対する化学療法後の外科切除の安全性と有効性

【ポイント】
◆R0切除困難な局所進行肝門部領域胆管癌に対して，化学療法を施行し，化学療法が一定の効果を示した症例に対して積極的に外科切除を行うという治療戦略は，安全であり予後改善に寄与する可能性がある．
◆R0切除が困難と判断する基準や，化学療法が奏効しR0切除が可能と判断する基準が，施設volumeによって大きく異なる可能性がある．
◆肝門部領域胆管癌に対するconversion surgeryに関しては不明瞭な点が多く，その安全性や有効性を前向きに検討していくことが重要である．
