肥満症の診断と併存疾患

【ポイント】
◆BMI 35以上は高度肥満と定義され，それに起因ないし関連する健康障害を有するものを高度肥満症と診断する．
◆高度肥満症で問題となる健康障害として，2型糖尿病，脂質異常症，高血圧症，さらにそれに起因する大血管障害，閉塞性睡眠時無呼吸症候群，肥満関連腎臓病，非アルコール性脂肪肝炎（NASH），心不全などがある．
◆減量・代謝改善手術の施行により，高度肥満症に伴う健康障害の多くは著明に改善し，なかでも2型糖尿病が劇的に改善するほか，生命予後も改善することが報告されている．
