胃切除術ではなぜoutermost layerが重視されるようになったのか

はじめに
　われわれは膵上縁郭清の際に，総肝動脈（CHA），脾動脈（SPA），左胃動脈（LGA）などの主要な動脈周囲には厚い神経線維の層とその外側にあるいわゆる「outermost layer」と呼ばれる至適な剝離可能層に沿った郭清手技を胃癌根治術の際の手術コンセプトとして重視している．本コラムでは，胃切除術ではなぜ「outermost layer」が重視されるようになったのか，「outermost layer」にまつわる色々をQ ＆ A形式で答えながら考察する．
