腹腔鏡下腹会陰式直腸切断術後に発症した会陰ヘルニアの1例

要旨
会陰ヘルニアは腹会陰式直腸切断術後に認める比較的稀な合併症である．会陰ヘルニアの予防方法や治療術式の報告は散見されるが，定まった見解はない．今回われわれは，腹腔鏡下直腸切断術後に発症した会陰ヘルニアの1例を経験したので，その予防方法や治療術式の文献的考察を含めて報告する．症例は76歳男性で，直腸癌に対して腹腔鏡下腹会陰式直腸切断術後10か月後に，会陰部の腫脹を主訴に来院した．CT検査にて小腸の会陰部への伸長を認め，会陰ヘルニアと診断した．開腹手術所見では，ヘルニア囊への小腸の癒着は認めず，Symbotex^TM composite meshを用い，新たな骨盤底を作成した．術後3年経過し，会陰ヘルニアの再発は認めていない．
