宿便性大腸穿孔

【ポイント】
◆宿便性大腸穿孔は，大腸に停留した硬便が大腸壁を圧迫し，粘膜の血流障害から壊死に陥り穿孔をきたすものである．好発部位はS状結腸が最多であり，大腸穿孔の原因のうち約3%を占める比較的まれな病態である．
◆高齢者，長期臥床者，慢性便秘者に多いとされ，症状は非特異的であることが多く，術前に診断することは難しい．慢性便秘者に安易な浣腸の施行は推奨されない．
◆穿孔部位の単純閉鎖などの術式は推奨されず，基本的には穿孔部切除と外瘻化が選択される．
