ISRは本当に有用な治療なのか—長期治療（腫瘍学的・機能的）成績からみたISRの有用性と問題点

【ポイント】
◆括約筋間直腸切除術（ISR）の適応基準は，①RMの確保，②DMの確保，③特殊癌症例の除外，④排便機能障害例の除外が原則である．
◆安全性・癌根治性は良好だが，術後排便障害を30〜40%に認める．
◆術後は癌のフォローだけでなく排便障害のフォローと治療が大切である．
◆適応患者を十分に選択すればISRは有用な治療である．
