術前CRTを施行した直腸癌に予防的側方郭清は推奨されるか

【ポイント】
◆進行直腸癌に対する骨盤内側方リンパ節郭清は，局所制御と生存率改善が期待される手技である．
◆腫大した側方リンパ節がない症例においては，術前化学放射線療法および直腸全間膜切除による治療後の側方領域局所再発は低頻度である．
◆術前化学放射線治療および直腸全間膜切除，選択的側方郭清を施行した症例の検討では良好な局所制御が報告されるが，今後，標準治療と比較した検証的な研究が必要である．
