両側側腹部圧迫法による腹部手術後の咳嗽時創痛の軽減効果

はじめに
　術後急性疼痛管理は，術後早期回復プログラムの概念などから，周術期管理における重要な事項に位置づけられている．術後疼痛は，頻脈，高血圧，高血糖，免疫抑制，局所血流や静脈還流の減少，血小板凝集といった生理学的反応だけでなく，恐怖，不安，うつ症状などの精神的作用を引き起こす^1〜4）．現在，術後早期回復プログラムのプロトコルでは，区域麻酔や局所麻酔，オピオイドあるいは非オピオイドによる鎮痛薬などにより多角的に鎮痛が行われている^5）．それでもなお術後疼痛は完全に克服されてはおらず，特に咳嗽時の創痛は高度である．800件の出版物と20,000人を超える患者を対象としたメタ解析では，手術患者の41%が中程度から高度の急性術後疼痛を経験し，24%が不十分な鎮痛であったと報告されている^6）．われわれは，両側側腹部圧迫法を考案し，本法による腹部術後の咳嗽時の鎮痛効果を報告した^7）．本稿では，本法についての臨床研究の概要を紹介する．
