食道胃接合部癌に対する内視鏡治療

【ポイント】
◆内視鏡治療適応を判断するうえで術前の深達度診断は重要であるが，食道胃接合部（EGJ）領域における診断精度は十分ではない．
◆本邦における多施設共同研究の結果から，リスク因子（脈管侵襲陽性，低分化型癌成分の混在，腫瘍径＞30 mm）のない深達度SM 500 μmまでの食道腺癌は，内視鏡治療後の経過観察が許容される可能性がある．
◆粘膜内癌か粘膜下層浸潤癌かで悩む食道胃接合部腺癌は，診断的な内視鏡的切除が許容されると考える．
