広範左側結腸切除後に一期的上行結腸直腸吻合再建ができた横行結腸閉塞性大腸癌の1例

要旨
症例は78歳，女性．閉塞性大腸癌の疑いで他院より紹介された．来院時，腹部膨満は軽度で嘔気がなかったため，まず禁食として中心静脈カテーテルを留置し点滴を用いた保存的治療を開始した．入院後腹部膨満が徐々に進行し嘔吐を伴うようになったため，本人の同意を十分に得て緊急手術を行った．開腹時腹腔内を検索すると横行結腸のほぼ中央に手拳大の腫瘍がみられ，S状結腸に直接浸潤して腸閉塞を呈していた．横行結腸からS状結腸の広範な左側結腸切除を行い，上行結腸と直腸を一期的に吻合再建した．吻合再建法は，上行結腸と回盲部を剝離授動して患者の左側に90度ローテーションさせ，上行結腸の右側縁と直腸の前壁を機能的端々吻合再建した．
