肝転移を有する胃癌の治療方針—NACかconversionか？

【ポイント】
◆胃癌肝転移は，多発例や，肝以外の非治癒因子が併存することが多く，切除の適応となる症例は限られる．
◆胃癌肝転移切除後の予後因子は，個数，大きさ，原発の深達度などであり，肝外転移をもつものは基本的に手術適応とならない．
◆化学療法の奏効により，R0切除が可能な場合は，conversion surgeryにより肝転移を伴う切除不能胃癌も予後の改善を期待できる．
