腹腔鏡下に摘出した60 mm大の腹腔内遊離体の1例

要旨
患者は55歳，男性．血糖コントロール不良の糖尿病のため，当院内科を紹介受診．精査目的の腹部CTで骨盤内に腫瘤を認め，腹腔内遊離体を第一に考えたが，孤立性線維性腫瘍（solitary fibrous tumor：SFT）や消化管間質腫瘍（gastrointestinal stromal tumor：GIST）などの可能性もあり，手術目的に当科紹介，本人とも相談のうえ，診断および治療目的に腹腔鏡下に摘出術を実施し，腹腔内遊離体の診断であった．腹腔内遊離体は，腫瘍との鑑別が困難な場合もあり，また，50 mm以上の巨大な腹腔内遊離体は腹痛や膀胱圧迫症状などの有害事象を起こしうること，発見時に症状がなくても今後症状をきたす可能性もあることから，診断的意義を含めて手術は妥当であったと考えられる．
