【ポイント】
◆高度肥満患者の腹壁ヘルニアに対し，腹腔鏡下スリーブ状胃切除術をはじめとする減量外科手術で術前減量を図り，二期的に腹壁瘢痕ヘルニア修復術を行うことで，手術成績が向上する可能性がある．
◆腹壁瘢痕ヘルニア修復術の術式は症例に応じて柔軟に選択する．
◆高度肥満患者に対するファーストトロッカー挿入は，左肋骨弓下からのoptical法が有用である．
