噴門側胃切除後再建におけるパウチの役割

【ポイント】
◆噴門側胃切除は上部胃癌における機能温存手術だが，胃全摘に対する術後QOLの優位性は確立していない．
◆空腸パウチ作製によって胃全摘後の症状が改善するエビデンスが示され，今後は噴門側胃切除後再建での導入が期待される．
◆パウチを併用する噴門側胃切除後再建では，逆流防止処置，腹腔鏡手術で施行し得る簡便性，縦隔内吻合に対応可能な術式の開発を要する．
