【ポイント】
◆幽門保存胃切除術は幽門側胃切除術に比して下痢，ダンピング症状，補食必要度の面で優れた術式である．
◆1/3〜1/2程度の残胃の大きさが至適であり，残胃幽門洞長は胃術後障害の程度に影響する．
◆吻合方法および迷走神経腹腔枝温存の有無が術後障害に与える影響の解釈は難しく，さらなる検討を要する．
