術後肝不全の予防

【ポイント】
◆肝切除後肝不全の定義は，International Study Group of Liver Surgery（ISGLS）によると，術後5日目におけるPT-INRの増加と高ビリルビン血症を認めるものである．
◆肝切除後肝不全の危険因子として過少残肝，肝機能不良，感染症，術中大出血，残肝血流障害などが挙げられる．
◆肝不全に対する特異的治療はないため，適切な術前評価と肝切除範囲の設定が重要である．
