大腸癌の炎症性リンパ節腫大に関連する臨床的背景—前向き研究からの探索的検討

要旨
大腸癌リンパ節転移の術前診断における，造影CT検査の検査特性および炎症性リンパ節腫大をきたす要因について臨床的に検討した．根治切除を予定された大腸癌（T2以深）症例を前向きに登録し，腫大リンパ節と病理所見を対応させ症例単位およびリンパ節単位で解析した．対象は194例，郭清リンパ節個数中央値17個［11〜24］（計3,656個）．リンパ節の短径7 mmをカットオフとした場合の感度・特異度は，症例単位では42.7%，67.2%，リンパ節単位では25.2%，98.3%であった．腫大リンパ節を認めた71例（36.6%）のうち，炎症性リンパ節腫大を39例（20.1%）に認めた．炎症性リンパ節腫大と腫瘍長径，深達度，閉塞症状，術前CRP値が関連していた．
