POINT
●マイルズ手術では癌の根治，機能温存のために，骨盤深部まで剝離層と切離ラインを選択しながら精緻な操作を行う必要がある．
●腹腔内から肛門挙筋を切離することで会陰操作が容易となるが，特に男性の前壁では複雑な筋構造を理解し尿道損傷に注意する必要がある．
●腹腔内からの肛門挙筋切離，骨盤底閉鎖，腹膜外経路でのストーマ造設などで，合併症リスクを低減できる可能性がある．
