ステロイドの短期投与で寛解した乳癌術後ペグフィルグラスチム誘発大型血管炎の1例—67報告例の検討

要旨
症例は68歳，女性．左乳癌，T2N0M0，StageⅡAに対して乳房全切除とセンチネルリンパ節生検施行．術後，ペグフィルグラスチム併用EC 4コース目のday 13に白血球とCRPの増加を伴い抗菌薬に反応しない発熱をきたした．CT検査にて大動脈弓部とその分枝血管の壁肥厚と周囲脂肪織の濃度上昇を認め，ペグフィルグラスチム誘発大型血管炎と診断．発症3日目から漸減期間を含めて2週間の短期のステロイド投与にて速やかに臨床症状，炎症反応やCT所見が軽快した．顆粒球コロニー刺激因子（G-CSF）製剤を併用した化学療法後の発熱や炎症反応の亢進時には，大型血管炎の鑑別のため速やかなCT検査を要し，治療はG-CSF製剤の中止とステロイドの短期投与が有効である．
