はじめに
　腹壁瘢痕ヘルニアは腹部手術の合併症の一つであり，腹部手術切開創をもつ患者のおよそ10〜15%に発生する^1）．正中切開創で最も発生頻度が高いが，横切開，傍正中切開，ポートサイトなどあらゆる腹部切開創に発生する．
　腹壁の機能は体幹動作だけでなく，呼吸，排尿，排便，咳嗽に大きくかかわっている．よって腹壁瘢痕ヘルニアの主症状は，腹部手術創の膨隆や痛み，腹壁の機能障害に起因する症状である．時に急性嵌頓や絞扼，皮膚壊死などにより緊急手術が必要になる場合もある．切開創に発生するため，どの部位にでも発生しヘルニア門のサイズや症状も千差万別である．このため単一術式のみですべての腹壁瘢痕ヘルニアに対応することは困難である．
　腹壁瘢痕ヘルニアの治療目的は，症状の改善による生活の質の向上と緊急手術の回避であり，診療ガイドライン^2）ではヘルニア門の縫合閉鎖とメッシュによる補強を強く推奨している．
