はじめに

　多嚢胞性卵巣症候群（PCOS）は多嚢胞性卵巣を呈し，内分泌学的には高LH血症と高アンドロゲン血症を特徴とする排卵障害である．不妊症を訴えるPCOS患者の排卵を誘発するに当たって，（1）クロミフェンに抵抗を示す症例が多く妊娠に結びつきにくい，（2）ゴナドトロピン療法を行うと卵巣過剰刺激症候群（OHSS）を起こしやすく多胎妊娠を発生しやすい，などの問題点がある．OHSSは過剰な排卵誘発により生じる医原性の疾患であり，近年のさまざまな誘発法の開発や胚凍結法の開発などによりかなり克服されたといえる．しかしOHSSの病態については未だに解明されていないところも多い．本稿ではPCOSの病態を考察し，OHSSをいかに予防できるかについて現在の概念や方法を中心に概説する．
