はじめに

　腫瘍マーカーは1975年にモノクローナル抗体の作製法が確立されて^1），さまざまな種類のものが発表されてきた．特に臨床応用されているものは糖鎖抗原関連腫瘍マーカーである．これはコア蛋白，母核糖鎖，基幹糖鎖，糖転移酵素，修飾構造（末端糖）などからなっている（図1）^2）．

　CA125抗原はコア蛋白であり，上皮性婦人科腫瘍で最も関連性のあるマーカーとしてよく知られている．成人において卵管上皮，子宮内膜上皮，頸管上皮，胸膜や心嚢膜中皮細胞，腹膜に存在する．表層上皮性卵巣癌に対する陽性率が高いが，子宮内膜症，胸腹膜の炎症性疾患，月経時や妊娠初期などでも上昇する．近年，CA125抗原分子がクローン化され，これが新たな膜結合型ムチンMUC16と称され，癌研究において現在注目を集めている^3）．これらについて最近の研究報告を含め解説する．
