はじめに

　胎児発育制限（fetal growth restriction : FGR）はすべての症例で発症機転が異なっているといっても過言ではなく，われわれは個々の症例ごとに適確な判断を要する疾患と認識している．そして，FGRの管理においていつも苦慮することは，胎児先天異常の有無と，いつ娩出すべきかの2点である．当センターは広域な診療圏の基幹病院であり，FGR症例の紹介も多く，紹介する側の医師の不安もやはり上記の2点に尽きる．早く娩出させればそれだけ人工的な早産をつくることになり，特に在胎30週未満においてはFGRそのものによる胎児障害以上に，児の未熟性に起因する神経学的障害が前面に出てくる可能性がある．また，妊娠期間を延長することは，子宮内胎児死亡の可能性のためにわれわれも患者も絶えず緊張を強いられることになる．さらに，FGRの管理に関しては諸家によりさまざまな見解があり，統一されていないこともわれわれを迷わせる要因である．

　本稿においては，当センター（周産期科・新生児科・小児外科）で採用している診断基準と管理方針を紹介する．
