月経異常による骨量減少

はじめに

　閉経後骨粗鬆症とエストロゲン欠乏との関連については近年研究の成果が著しく，数多くのことが明らかにされてきた．一方，閉経前の卵巣機能不全についても骨量減少を引き起こすことが推測され，特に思春期からの無月経は，最大骨量の獲得に影響を与えて将来の骨粗鬆症が憂慮される．

　エストロゲンは破骨細胞，骨芽細胞いずれにも作用し，破骨細胞による骨吸収と骨芽細胞による骨形成との間の平衡関係（リモデリングバランス）を維持することで骨量を保つと考えられている．エストロゲンが欠乏すると，骨代謝回転が亢進して骨吸収が優位になって骨量の減少を引き起こし，皮質骨より海綿骨での減少が著しくなる．閉経後ばかりでなく思春期あるいは性成熟期にあっても，エストロゲンが低下して無月経になるとその合併症として骨量の減少を招くことが指摘されている．

　本稿では，思春期から性成熟期を通じての卵巣機能不全（月経異常）と骨密度に関する知見を述べる．
