子宮外妊娠に対するmethotrexate全身療法後に発症した癒着性イレウスの1例

はじめに

　経腟超音波断層法や低単位human chorionic gonadotropin（以下，hCG）検出薬などの補助診断の進歩によって子宮外妊娠の早期診断が可能となり，妊孕性温存を考慮した初期治療としてのmethotrexate（以下，MTX）による薬物療法も実施されるようになっている^1～4）．しかし，その治療適応基準は未だ確立していないのが実状であり，治療経過不良例も散在していると推定される．その多くは治療中の卵管破裂などによる腹腔内出血と推定されるが，今回われわれは，MTX全身療法から1か月以上経過したあとに骨盤内流産血腫を原因とした癒着性イレウスを発症した症例を経験したので報告する．
