よい精子とは

　よい精子とは十分な受精能と完全な父親の遺伝情報を備えた精子であり，そのような精子をより多く回収することが要点となる．精子選別法として，現時点では運動性が良好な精子を選別回収する方法が採択されている．

基礎知識

　DNAダメージと核タンパク

　精子のDNAには核DNAとミトコンドリアDNAがあり，1998年ころからそれらのDNAのダメージ（断片化）の有無と受精能，胚発生能との関連が報告されている^1）．最近では精子DNAダメージと精子核タンパク（プロタミン）異常との関連が注目されている^{2, 3）}．射出精子の核タンパクはプロタミンで，体細胞の核タンパクであるヒストンと異なり，DNAの結合を固く補強する役割を持つ．よって，プロタミンの異常はDNAの結合を脆弱化する可能性があり，ダメージに直結する．精子DNAダメージへの精子処理の影響については，精子の培養時間が長いほど，また遠心処理が多いほどダメージが強くなる^4）．精液処理については無処理が最もダメージが軽微で，遠心洗浄，無洗浄のswim─up法，洗浄精子のswim─up法の順にダメージが強くなる^4）．密度勾配遠心法はswim─up法よりDNAダメージが少ないといわれる^5）．精子DNAのダメージが多いケースではIVFやICSIでの胚発生，特に胚盤胞への発生率が低下する^{3, 5）}．しかし，現時点では精子DNAダメージは妊娠率に影響しないと報告されている^6）．
