創部膿瘍と思われた子宮頸癌皮下転移の1例

症例

　患者は34歳の主婦（2回経妊・2回経産）である．約半年間の不正性器出血を主訴として，近医を受診し子宮頸癌の診断にて当院を紹介された．MRI所見では頸部に52×48mmの腫瘤が認められ，傍結合織への浸潤が認められたが，明らかなリンパ節腫大や遠隔転移巣は確認されなかった．血中腫瘍マーカーはSCC＜0.5ng/ml，CEA 0.8ng/ml，CA19─9 11.1U/mlと正常範囲内であったが，CA125値のみが63.9U/mlと軽度上昇していた．

　子宮頸部扁平上皮癌（非角化型）IIb期の診断のもと，DJ療法（ドセタキセル80mg＋カルボプラチン800mg）1コースによる術前化学療法（NAC）を施行した．NACにより局所病巣は36×32mmへと縮小し，CA125値も9.4U/mlと正常化し，広汎性子宮全摘術ならびに骨盤内リンパ郭清術を施行した．摘出した腫瘍サイズは38×27×10mmで，術後病理診断では深達度7mm，リンパ管侵襲（＋），血管侵襲（－）であった．患者の意思により術後追加療法を行わず，外来フォローとなった
