はじめに

　初経年齢の低齢化，出産年齢の高齢化，小子化という女性のライフスタイルが子宮内膜症のリスク因子とされているが，これは卵巣癌が増加している背景そのものでもある．内膜症は若年～性成熟期婦人に好発する月経痛を特徴とする良性疾患であり，不妊症と密接に関連するため，実地医家や不妊・内分泌を専門とする産婦人科医で管理されることが多い．当然，“腫瘍”という概念に乏しく，疼痛（月経困難症）や不妊という観点から手術の適応が決定されてきた．その一方，内膜症性（チョコレート）嚢胞と卵巣癌の合併例の報告が蓄積されてきた．近年，「チョコレート嚢胞はがん化することがあり，年齢や腫瘍径に相関してがん化率が上昇すること」が報告され_{1, 2）}，子宮内膜症取扱い規約（2004年版）には「endometrial cyst合併卵巣癌に対するガイドライン（卵巣チョコレート嚢胞の悪性化）」という項目が記載された_3）．すなわち，子宮内膜症は“腫瘍”としての経過観察や治療の必要性が認識された．卵巣チョコレート嚢胞は閉経したから大丈夫ではなく，卵巣嚢腫と同様に経時的に悪性化することを認識せねばならない．

　本稿では，悪性化という観点から，その発生，経過観察手段，治療のタイミング・方法について，婦人科腫瘍の治療を担当している立場より1つの見解を述べる．
