はじめに

　子宮内膜症は，子宮内膜組織あるいはそれに類似した組織が異所性に発生または生着し，発育する疾患である．月経痛，性交痛，あるいは慢性骨盤痛などの疼痛原因となり，女性の生活の質（QOL）を著しく損ない，日常生活に多大な影響を与えるものである．個々の患者の多彩な疼痛症状を把握することは，的確な治療法を選択するうえで非常に重要なことである．しかし，子宮内膜症は腹膜病変，深部病変，卵巣子宮内膜症性嚢胞，さらに子宮腺筋症と異なった病態を呈する疾患であるため，疼痛原因を一元的に論じることは難しい．そこで本稿では，子宮内膜症と疼痛との関係を各々の病態に即して整理し紹介する．
