不妊に対して薬物療法は有効か

はじめに

　子宮内膜症患者の30～50％に不妊を認めることから，子宮内膜症が妊孕性を低下させる疾患であることは周知の事実である．本疾患における妊孕性低下には，付属器癒着などによる解剖学的異常と腹腔内環境の異常による受精障害・胚発育障害などの機能的異常が関与していると考えられている．子宮内膜症の治療は手術療法と薬物療法に大別されるが，手術療法は解剖学的異常を修復し，さらに病巣除去により腹腔内環境を改善できる．その結果，不妊合併症例の妊孕性改善に対する治療効果が期待でき，実際にその有用性が示されている．一方，薬物療法は，疼痛改善効果は認められるものの，妊孕性改善に対する効果については疑問視されている．

　本稿では，薬物療法の妊孕性改善に対する効果に関する諸家の報告を紹介し，子宮内膜症合併不妊における本治療の位置づけについて述べる．
