2) 体癌

1　病因・病態

　子宮体癌の多くは，プロゲステロンの拮抗がない内因性，外因性のエストロゲンの曝露により，発生母地である子宮内膜が異形成から癌へと進行し発生すると考えられている．このような過程で発生した体癌は全体の約9割を占め，核異型のgradeが低く，類内膜癌で，予後のよいtype Iに分類している．残りの約1割を占めるのは，核異型のgradeが高いか，漿液性腺癌もしくは明細胞腺癌で，p53抑制因子の突然変異がある予後の悪い群であり，type IIと分類している^1），表1に子宮体癌の増悪因子・軽減因子を示す．

　また，遺伝性癌症候群の1つであるhereditary non─polyposis colon cancer（HNPCC）は，右側大腸癌，子宮内膜癌，その他の癌が発生する症候群で，修復遺伝子の転座が病因である．
