5. 男性不妊

1　病因・病態

　不妊外来を訪れるカップルに占める男性不妊の頻度は40％～50％に上る．精液検査の結果，精液量・pH・精子濃度・精子運動率・精子正常形態率・抗精子抗体などがWHOによる基準値^1）を下回る場合，さらに1～2回の再検査を行い，一度でもすべての項目に合格すれば男性因子なしと判定する．

　男性因子ありと判定した場合，精液所見不良となる原因を特定するため，以後は泌尿器科学的精査に委ねることになる．男性不妊症の原因はその病態により，表1に示すように造精機能障害，精路通過障害，副性器障害，性機能障害，免疫性不妊症，その他に分類できる．
