PCOSの診断基準―日本の基準は欧米とどこが違うのか

はじめに

　多嚢胞性卵巣症候群（polycystic ovary syndrome : PCOS）は産婦人科分野では高頻度にみられる内分泌疾患であるが，原因がいまだ明らかではないこと，またその病態も多様であることから診断基準そのものが普遍的なものには至っていない．さらに，最近ではインスリン抵抗性の問題が着目され，全身疾患としての側面からPCOSを捉えようとする動きもあり，診断基準の見直しについても言及されてきている．本稿では日本と欧米における診断基準について，その違いと問題点について概説する．

　PCOSは1935年にSteinとLeventhal^1）により，卵巣腫大，男性化徴候，無月経，肥満を症候とする疾患概念としてはじめて報告された．以降，その病態や病因についてさまざまな検討がなされ，内分泌学的には高LH血症，高アンドロゲン血症を示し，卵巣では多くの閉鎖卵胞を含む卵巣腫大，白膜肥厚が認められることが示されている．最近ではインスリン抵抗性の合併頻度が高いことも報告^{2, 3）}され，1つの重要な病態として着目されてきている．
