はじめに

 多嚢胞性卵巣症候群(polycystic ovary syndrome : PCOS)にみられるインスリン抵抗性,すなわち高インスリン血症はPCOSの病態の1つというだけでなく,PCOSの発症そのものに深くかかわっていることが明らかとなってきた.肥満はインスリン抵抗性を伴うが,肥満を合併したPCOSは肥満だけの症例よりインスリン抵抗性が強く,非肥満のPCOSでもインスリン抵抗性がみられる1, 2).これらのことから,PCOSのインスリン抵抗性はPCOSに本質的な病態であると考えられるようになった.本稿では,インスリン抵抗性からみたPCOSの病態を最近の知見を交えながら解説する.
